We are thrilled you are interested in joining us for rehab at NFPT!  To help your first appointment run as efficiently as possible, please download and fill out the new patient forms listed on the left, and bring them to your first visit.  Note, there is one additional form that Aetna and United Healthcare plans are required to submit.  Please contact our office if you have any questions.

Frequently Asked Questions

What should I expect at my first visit?

A licensed physical therapist will personally meet with you to discuss your needs and perform a thorough physical therapy examination to develop a proper plan of care that meets your goals.  Each patient case is unique, but the first appointment can last between 30 minutes to 1 hour.

What should I wear to therapy?

Please plan to wear comfortable clothing that allows you to move easily.  If a lower extremity is involved in your case, please wear something that will allow the physical therapist to access and assess that region.

Do I need a prescription from my doctor? 

We recommend patients first investigate their specific benefits to confirm all requirements.  For insurance filing purposes, a prescription from your doctor is necessary at the time of the first visit.  Certain benefits plans also require a referral from your primary care provider.  There are other scenarios where a doctor referral and/or prescription are not necessary.  If you have any questions, please feel free to call our office for guidance and clarification.

What methods of payment are accepted?

We accept all major credit cards, as well as cash and personal checks.

How many times will I have to come to physical therapy?

Based on your specific needs, this will be determined by you and your physical therapist at your first appointment.
